DRUG FREE WORKPLACE CONSENT AGREEMENT

I understand that Surgery Partners has a policy to maintain a drug free workplace. In accordance with the Drug Free Workplace Policy, all prospective employees must successfully complete a
drug screen urinalysis as a condition of employment. Further, current employees may also be required to submit to drug screening, as a condition of continued employment, as specified under the guidelines of the Drug Free Workplace Policy.

I hereby acknowledge that my submission to a drug screen urinalysis is a term of employment and will be administered at no cost to me. All medical information obtained as a result of this test will be classified as confidential with the exception of the positive or negative result. I understand that a positive drug screen result will preclude me from any further consideration of
employment with Surgery Partners unless reapplication requirements are met within the terms specified in the Drug Free Workplace Program. I also understand that, once employed, a positive drug screen result may subject me to disciplinary action up to and including termination.

In consideration of the above information, I hereby agree to submit to a drug screen urinalysis and authorize the release of the results to representatives of Surgery Partners. Also, I hereby
release directors, managers, officers, employees or other representatives of Surgery Partners from any liabilities, damages, judgments or causes of actions, including costs and attorney fees
before trial, at trial or on appeal, that result from this policy or testing procedure as applied to me.

DATA PROTECTION STATEMENT

The Data Protection Act 1998 ("The Act") sets out certain requirements for the protection of your personal information against unauthorized use or disclosure. Except to the extent we are required or permitted by law, the information which you provide in this application form, and any other information obtained or provided during the course of your registration and application ("the information") will be used solely for the purpose of assessing your application. If your application is unsuccessful or you choose not to accept any offer of employment we make, the information will not be held for longer than necessary, after which time it will be destroyed, although relevant information will be retained in the longer term to facilitate our equal opportunities monitoring. If your application is successful, the information will form part of your employment file and we will be entitled to process it for all relevant purposes in connection with your employment.
