ROCKET FARM RESTAURANTS, LLC

RELEASE AND WAIVER OF LIABILITY

For and in consideration of potential employment with Rocket Farm Restaurants and its affiliates I hereby voluntarily waive, release and discharge any and all claims of negligence and other damages relating to personal injury, death, or other damages that may occur during my temporary performance at Rocket Farm Restaurants and affiliates. To the best of my knowledge, I am in good physical condition and fully able to participate in this trial performance. I am fully aware of the risks and hazards connected with participation in this event, including physical injury or even death, and hereby elect to voluntarily participate in said event, knowing that the associated physical activity may be hazardous to me and to my property.

I assume all responsibility for personal injury, death, or property loss from any cause whatsoever. I agree to give up and make no claim against Rocket Farm Restaurants or any of its affiliates, their officers, directors, agents or employees for any injury to myself or others, death or property damage regardless of cause, including alleged negligence or fault. I agree that my signature on this waiver and release confirms absolutely my agreement to be bound by all these terms and others posted.

I further understand that this waiver is intended for the release of temporary liability as I, the applicant, perform the applied for job tasks and objectives. If hired as an employee of Rocket Farm Restaurants and its affiliates this wavier will be repealed, lifted, and considered no longer valid as I, the applicant, will have become a legitimate and legal employee of Rocket Farm Restaurants and affiliates, and will have all the rights and responsibilities provided to me by the Department of Labor and the Rocket Farm Restaurants’ Employee Handbook.
