APPLICANT CERTIFICATION & AGREEMENT

I understand that completion of this application does not imply that I will be interviewed or hired. I certify that the information provided on this application and any resumé and any other materials submitted by me to Centra is correct and complete to the best of my knowledge. I understand that providing false or misleading information in this application or any resumé will result in a decision to terminate the consideration of my application. In addition, I understand that if I am hired and Centra later discovers that I provided false or misleading information in any application, resumé or other materials my employment will be terminated regardless of the date of discovery. “False or misleading information” includes, but is not limited to, any material information that I omit from this application, resumé, or other materials. I also understand that if I am hired by Centra, my employment will be at-will, which means that it will be employment for no specified period and that it may be terminated by me or Centra at any time, with or without cause. I understand that, if hired, my at-will status shall not change unless a written agreement signed by Centra’s President is entered into with me.

I also understand that information concerning my past record will be sought from previous employers and other sources and I hereby release from all liability or damages those individuals, corporations or organizations who provide such information. I understand any such information provided shall become the exclusive property of Centra. I also understand that any employment offer that I receive may be contingent on Centra receiving an acceptable verification of the information on this application and any resumé and other materials.

If I am hired, I agree to conform to Centra’s policies and rules. I acknowledge that Centra may change these at any time at its discretion and without prior notice to me.

In consideration of Centra’s review of this application, I hereby agree that: (a) any claim, action or lawsuit against Centra, its employees, agents or representatives (“Centra Group”) arising out of or relating to the hiring process, my employment, or termination of employment, including, but not limited to, any claim for employment discrimination or arising under any Local, State, or Federal civil rights law, must be filed in a court of competent jurisdiction or with any State or Federal agency having jurisdiction over such matter within 180 calendar days of the event giving rise to the claim or be forever barred, and I knowingly and voluntarily waive any statute of limitations or other period to the contrary; and (b) I knowingly and voluntarily waive any right that I may have to a jury trial in connection with any claim, action or lawsuit against Centra Group relating to the hiring process, my employment, or the termination of that employment, including, without limitation, any claim or action relating to employment discrimination. This means that any such claim or action will be heard by and tried before a judge. I am unequivocally waiving each of the above-described rights knowingly and voluntarily.

In compliance with the Immigration Reform and Control Act of 1986, Centra will not hire individuals who are not lawfully authorized to work in the United States. Accordingly, if hired, I understand that I will be required to submit proof of my legal right to work in the United States and that Centra cannot continue my employment if I am unable to provide such proof.

I authorize any of the persons or organizations referenced in this application, resumé or other materials to provide Centra with any and all information regarding my previous employment, education or any other information that they may have, personal or otherwise, with regard to any of the subjects covered by this application and/or resumé or other materials, and I release all such parties from all liability for any damage that may result from furnishing such information to Centra.
