I certify that the above information is true and complete. False statements or omissions from this application shall be sufficient cause for denial of employment or termination, if discovered after employment. Any resume attached to or submitted with this application will be incorporated in and considered a part of this application and subject to all provisions stated herein.

I declare my answers to the question on this application are true, and give Avadim Holdings, Inc the right to investigate all information given and to secure additional information if necessary. I understand that an investigative report may be made whereby information is obtained through personal interviews with third parties. This information may include, but is not limited to verification of previous employment and employment references, verification of education including transcripts, criminal reports, etc. I hereby release all persons, companies and corporations furnishing such information and release the company, subsidiaries, employees and agents from any liability arising from any actions taken related to employment investigations. Please note that conviction records will not necessarily bar applicants from employment.

I acknowledge that should an offer be extended to me, that it would be contingent upon my taking and successfully passing pre-employment drug test and background check. I further acknowledge that refusal to undergo such test or not passing such test, will result in denial of consideration for employment by the company.

I understand that the employment with Avadim Holdings, Inc., is employment at will; that is, either party, for any reason or no reason may terminate such employment at any time.
